Medication Form
Please use the following link to print the medication form. This form is required for medications to be administered at school.
If your student must take medication during school hours, a medication form must be filled out by the parent or guardian. The medication, either prescription or over the counter, must be brought into the clinic along with the medication form. A form can be obtained at the clinic or printed from this web page. If you have any questions, please contact the nurse.
